    Registration Contract, Waiver & Release

    To participate in the Thundering Lions Kung Fu LLC Free Trial, please read the below participation waiver and program requirements. The responsible party must read and understand the entire contract, and fully agree to comply with our rules and policy before clicking "Accept & Continue." Filling in the required fields below shall legally affirm the following:

    I do hereby agree to participate in the Thundering Lions Kung Fu LLC Free Trial Class (referred to as Thundering Lions Kung Fu) located online at www.thunderinglions.com.  I am aware that the program will require exercise and that exercise can be physically stressful and, in rare instances, can even be harmful. I am also aware that anyone who smokes, has ever had elevated blood pressure, is over the age of 45(for men) and 55(for women), who does not currently exercise, or has ever had cardiac (heart) problems, is overweight, or is susceptible to orthopedic problems is more at risk while exercising.

    I understand that my participation in the Thundering Lions Kung Fu is voluntary and at my own risk. On behalf of myself, my spouse, my heirs, and my assigns, I hereby release Thundering Lions Kung Fu, and each of their affiliates and subsidiaries, and each of their respective agents, directors and employees, from and hold them harmless against, any liability and claims for personal injury, loss, damage, or harm that I may sustain or incur in any way arising out of or in relation to the their negligence or my participation in the Thundering Lions Kung Fu Program.

    The information provided in this program is not intended to be a replacement for medical advice, nutritional counseling or exercise prescription. If I have any questions or concerns about my safe participation in this school/program, I take full responsibility to consult with my physician prior to and during my participation.

    I understand that participation in this program is an acknowledgement of my agreement to this participation waiver. I hereby execute this Waiver and Release form permitting my minor child and / or myself to participate in the Company's program.
